Dedicated Providers for Every Facility

  • Skilled Nursing Facilities (SNFs) have a dedicated Advanced Practice Provider (APP) and a physician.

  • Assisted Living Facilities (ALFs) have a dedicated APP, with an option of a Medical Director.

Insurance-Based Billing

  • All patient visits are billed directly to insurance for convenience and accessibility.

24/7 On-Call Medical Support

  • Monday through Friday (8 AM - 5 PM): Patients have access to their dedicated provider.

  • After Hours (5 PM - 8 AM) & Weekends: On-call APP or physician available to order tests and medications as needed.

Seamless Communication & Documentation

  • Every visit is documented and shared with facility staff to ensure transparency and coordinated care.

Medication Management & Safety

  • Upon admission and with each visit medication reviews are performed to ensure medical necessity, dosage accuracy, and to decrease polypharmacy.

  • Providers lead antibiotic stewardship programs to reduce unnecessary antibiotic use, minimize harmful side effects, and decrease the risk of antibiotic resistance.

Facility Leadership & Staff Education

  • Medical Directors:

    • Attend QA meetings and assist in achieving high star goals.

    • Provide education and training for families, facility staff, and other medical professionals.

    • Facilitate teamwork and ensure appropriate, person-centered medical care.

Family Engagement & Support

  • Families are encouraged to participate in their family members’ care from the initial admission and continuing throughout their facility residency.
